Repeat Prescription Request

This page is for patients who are currently under Mind Horizons’ care (i.e. who have been seen by a Mind Horizons psychiatrist in the last 3 months at Mind Horizons) and for medications that they are currently (or have recently been) prescribed. If you are wanting to start a new medication, we suggest you book in a follow-up appointment with your regular psychiatrist as prescriptions will only be issued if they are clinically appropriate and it is usually inappropriate to start new medications without discussing this with a patient so they can give adequately informed consent.

If your prescription request is not clinically appropriate, your psychiatrist will contact you via email to let you know what his recommendations are in lieu of issuing the prescription.
